Stress incontinence is the involuntary loss of control of urine associated with anything that causes people to tense up their stomach muscles – coughing, sneezing, lifting, exercise – things like that. It often affects old women.
23-45 percent of women have experienced incontinence, and about 7 percent of them have serious incontinence symptom, 50 percent of whom are stress incontinence. The correlation factors are: (1) Age: The morbidity of female incontinence is higher and higher with the increase of age. It is most often seen in women of 45~55 year-old; (2) Breeding: times of breeding, the age of first breeding, ways of delivery, the size of fetus and incidence of pregnancy incontinence are all associated with postpartum incontinence; (3) Obesity: Fat women have higher stress incontinence incidence. And if they loss weight, the incidence could be reduced.
According to the severity, we classify stress incontinence into three grades: (1) Mild grade: There is no incontinence when this kind of patients are doing normal exercise or at night. And when their abdominal pressure is increased, sometimes they will have incontinence. There is no need to use urinal pads; (2) Moderate grade: There is frequent incontinence when they tense up their stomach muscles and stand up to exercise, and they need urinal pads; (3) severe grade: Once the patients stand up or change their decubitus position , they will have incontinence, which severely affects the patients’ quality of life and society activity.
Treatment: (1) Pelvic floor muscle training: Voluntarily contract pelvic floor muscle for 2~6 seconds, then relax for 2~6 seconds. Repeat this exercise for 10~15 times, and take this training above 3~8 times everyday for more than 8 weeks. (2) Oral pharmacologic treatment: We can use drugs like selective α1 adrenoceptor agonist to increase the pressure of urethra when it is close. (3) Surgical treatment: Indications: 1.When conservative and pharmacologic treatments are not effective, or the patient could not insist and tolerant this therapy. 2. Moderate or severe stress incontinence, which affects the quality of life severely. 3. The patient requests good quality of life.
The surgical approach: Tension-free vaginal tape (TVT). The effective rate of TVT is 84~90 percent in short terms. TVT is the most popular surgical approach of female stress incontinence as this operation has good therapeutic effects as well as less invasions and complications.
